Transcript Request

To request a transcript, please complete the attached Google form or submit a paper request in the Counseling office. Processing takes 3-5 days. 

• For Graduates from 2004 to Present: PHS can fulfill your transcript request.
• For Graduates from 2003 and Earlier: Requests must be directed to the MSBSD Office of Instruction @  Grad year 2003 and Prior Transcript Request

Final Transcript Requests:

Graduation packets, distributed during practice, include transcript request forms. Colleges and trade schools will need a final transcript, and it is the student's responsibility to submit the request. Staff will leave at the end of May for the summer, so please submit your request before then. After that, you will need to contact MSBSD.
