Transcript Request
To request a transcript, please complete the attached Google form or submit a paper request in the Counseling office. Processing takes 3-5 days.
- For Graduates from 2004 to Present: PHS can fulfill your transcript request.
- For Graduates from 2003 and Earlier: Requests must be directed to the MSBSD Office of Instruction @ Grad year 2003 and Prior Transcript Request

FINAL TRANSCRIPT REQUESTS:
Transcript request forms are included in the graduation practice packets. Colleges and trade schools will require a final transcript. This is the student’s responsibility
